Anyone ever had a character with glasses / monocle / eye disorder, and had to deal with their lack of perception? Yea, I know that people with those problems didn't live long in the Middle Ages, but in the rulebooks, there are definetely pictures of gnomes with glasses. Suggestions?

Yes I did, and during combat I allways gave a 5 percent chance they would come off.
Or during an attempt to flee, or climbing or doing general Dungeoneering stuff I would also give it a 5 percent chance of coming off.
Now if they came off I had a set modifyer.
-1 on to hit, and also a +10 percent chance of tripping traps.
Anything else I handled on a per event basis.

I suppose its depends on how recent the injury is. A character recently blinded should be penalised more than a character who has suffered for several years.
I always gave a -10 to hit and +1 saving throw penalty for the first week and then decreased the to hit penalty by one per week. The saving throw penalty disappears after 5 weeks of gameplay.
Harsh and not necessarily realistic but fun to play.
