I have to say that my group uses very little of the "official" WoD setting or metaplot. We've never played through a published adventure (I have the first Giovani Chronicles and am tempted to use it sometime, but the oppurtunity hasn't come up).
While I allow thinblooded and dhampir characters it has more to do with my owning Time of Thin Blood than the year of the Reckoning storyline. I haven't put in the Week of Nightmares or the destruction of the Sabbat Tremere or the lifting of the curse on the Assamite, and might never use those events.
I run three chronicles, all on the theme of what a city in the modern era that is ruled by a supernatural being might be like.
One is a city that's been taken over by an army of vampires and ghouls who lock the humans up in the city's jail and use them like cattle for food. Then the city is taken by the Sabbat. The PCs are trying to free it from the Sabbat and restore it to something like normal.
The second is Verona. This city has been ruled by a vampire who has acted as a sort of immortal god-king for around four thousand years. The mortal populace actually likes this situation. But rebellion has broken out among the childer of Esclias (the prince). Some support him and others seek to supplant him. The PCs can be on either side of the struggle.
The last is Island City, a place so rich with magical energy that supernaturals positively flock to it, leading to a rather bizarre imbalance where almost everyone is if not a major supernatural like a vampire, werewolf, mage, or changeling they are a one step removed type like a ghoul or kinfolk or mortal with numina. And yeah, the basic storyline here is about a war ver control of the city.
